You have to apply Offline - with the help of RM or BM. It's a very tedious job and they can refuse you.
Documents: PAN, AADHAR, PHOTO - they can ask for an account of the add-on member as well (customer id).

Pan, photo and DL only is valid to apply addon card. No other address proof accepted. Not even asdhaar.
