Credit card LE and upgrade documentation

I had filled an online form from HDFC for LE and card upgrade, and got the response as attached in this thread. They are asking for salary slips, statement which is fine but they are also asking a signed form to be courrier to their bank. Is this really required or can I download the form, fill it, sign it and send the scan copy along with payslips and statement?

You can mail to grivenance directly with form filled and income documents attached theu consider it also.

In the form, I can just see the price for Diners Club Black, My diners Privilege card is LTF. Not sure what to select here to get it upgraded to DCB LTF.
