Getting error while applying for Cashback card via sprint

I m trying to apply for SBI Cashback credit card via Sprint. But in the personal details section when I try to submit details I am getting error - "Something went wrong. Please try again."

What I think the possible reason for this could be is my last name. I don't have last name in my legally documents but here in the card application form the last name field is mandatory. If I put a blank space or my last name also it appear As Undefined which could possibly be the reason for the error while submitting the details.

Has anyone here come across this issue or any idea what could be the work around for this issue. Attaching the error screenshot for reference.

Not accepting it. Says only alphabets allowed. It accepts blank space in last name but when moving further to other details full name appears as - Kuldeep Undefined.
