How are milestone spends calculated?

Sadly my card portfolio now has too many non- LTF cards and it's getting tough to keep track of how far I'm from my milestones. So, does anyone know how are the annual milestone spends calculated? Of all cards i own, only kotak shows it clearly on the app. My questions are below.

1) is it sum total of all bills generated in last 1 year?
2) does the calculation start from day of card activation or next calendar month?
3) how are emi spends added to milestone, if emis fall in 2 different milestone years?
4) is it affected if i pre-pay before bill generation.
5) what happens if a transaction is eligible for cashback?
6) also has anyone tried calling the bank and get info about how far they are from milestone?

1. Total expenses less cancelled/failed transactions
2. From card issuance date
3. Spend in the card anniversary year are captured, need to check, I have never used EMI on card
4. No
5. Doesn't matter, it is still counted
6. No, I track my expenses periodically so know how far I have reached, but good idea to confirm with Bank
