How to apply for priority pass regalia card

Is this link enough for applying priority pass for regalia card or something else has to be done?

After applying did not get an msg or acknowledgement. Not sure if it's enough only to fill form n they automatically deliver the card to my address

I haven't linked it actually on my spouse HDFC Regalia Gold Card. It automatically got issued and linked.

No this is for my friend actually. My friend did not get priority pass automatically when regalia card was delivered

I tried this way last month for my relative, it didn't work. (I asked this last month)
Later the card owner contacted customer care and raised a request, it got delivered last week.
