How to Apply for YES Bank Add-on Credit Card
You can apply for a YES Bank add-on credit card online as well as offline:

1. Apply for YES Add-on Credit Card Online via YES Online​

The primary credit cardholder can log into the YES Bank credit card netbanking account and apply for the ‘add-on credit card’. He/she needs to enter details of the family members they would be adding as a secondary cardholder. Once the details are filled, the YES add-on credit card application can be submitted.

2. Apply for YES Add-on Credit Card Offline by Visiting the Nearest YES Bank branch​

You can also apply for a credit card by visiting your YES Bank home branch. Get in touch with a bank executive and raise your request to apply for a secondary YES credit card. Once all the required credit card documents are submitted along with the application form, your YES Bank add-on credit card application will be processed.

Eligibility & Documents Required to Apply for YES Bank Add-on Credit Card​

1. Eligibility for YES Bank Add-on Credit Card​

To be eligible for an add-on card, the applicant must be a family member of the primary cardholder. The following relations of the primary credit cardholder can apply for a YES add-on credit card.

• Father
• Mother
• Brother above 18 years
• Sister above 18 years
• Wife
• Children above 18 years

2. Documents for YES Bank Add-on Credit Card​

• Identity Proof: PAN card, Aadhaar card, Government-issued ID card
• Address Proof: Passport, driving licence, Aadhaar card, Ration card

It is not working.where to fill form after login

Just now, I got add-on for my wife.

Process only through branch only, and documents required are:
1. Add-on form with primary holder signature
2. Signed PAN & aadhar (latest downloaded from madhaar) of add-on member
3. Aadhar consent form, aadhar digital verification with QR code and then signed by add-on member

Once above documents are submitted then 1-2 weeks time is required to get add-on card. I have to submit above document twice because aadhar copy was old downloaded version.
