Incorrectly updating the dates
Entering an amount that the system does not recognize as valid
Providing an incorrect account name
These are the most frequent causes of failure. I have also experienced some of my disputes falling into the 'failed' category. However, there is no need to worry about this. Simply resubmit the dispute correctly, and it will get res olved.
