Update:- Please find the process below. It's very simple.

1. Download the Ad-On card form from Axis website or asked bank official to send it to ou.
2. Fill up the form along with the photo of Ad-On card holder.
3. Get one Xerox of PAN and AAdhar car dof Ad-On card holder ans get it signed by him/her.
4. Submit it at the bank.
5. They will first refuse and say Ad-On card application is on hold. But you can tell them customer are team asked them to give it to bank.
6. Take the service request no. and keep track of it, if it is beyond 7 working days.

Is this gonna work for entry level cards like Flipkart Axis, Axis Ace and My Zone?

Wish I had read this before applying last week. Followed all the steps but didn't ask for a service request no. and I suspect my application is still rotting in some drawer!

In my case, I got service request number after 4-5 days and card from 5 days thereafter.
Totally depends on the branch. You can ask service request number even now.
As a good practice, better to keep a copy of application form just in case of any follow up query.

See the reply I received when following the exact same procedure and applying for an Axis Reserve add-on card at the branch.

This is a really senseless rule. Axis Bank thinks that my brother, who is a paying 10k annual fee for Vistara Infinite should cancel his card to avail of an add-on card on my name. 🤦‍♂️how does this benefit the bank?!
Can someone suggest what I should do now?! Anyone from Axis Bank on this forum who can throw some light on "UCIC 2 guidelines"?

Yea
I have written about the same to Axis MD
Let’s see what he replies on that
PNO said that the technical infrastructure does not allow it

UCIC: Unique Customer Identification Code. So basically the bank might have a 1:1 relationship with the add-on applicant and hence they might not be able to assign another card to the same UCIC because UCIC should always be unique and cannot be combined from multiple sources as per RBI.

I had some issues with Axis when I still had an account with them. The branch staff whom I know told me this. I am not sure to what extent this is true because as far as my understanding goes, UCIC is unique across banks, but no idea if it is also within the same bank.

It is correct actually. This is such a weird policy by Axis Bank. My wife's add-on card was rejected due to the same reason. Later I cancelled her primary card which was useless. Now I will apply through this procedure.
