I've been trying to get the upgrade form and the CC guys shared it with me over the email however the link doesn't seem to be working. Wondering if anyone has access to the upgrade form? It'll be great if anyone can share it across with me

Verify the link, might be fraud. Mostly upgrade should happened via HDFC mobile app or net banking account.
