As usual HDFC bank RMs, BMs, Manager are clueless.

Login to HDFC Bank
> To Pay > Credit Card

>select the Credit Card for which you want the Addon Card..


> then you will have a AddOn Cards at right bottom there you will have a option for requesting a Addon card

This UI was by default for new a/c created. One of my family member got this ui in march end itself!
