How to change DOB, EMAIL-ID, wrongly filled by the agent?

I applied for SBI Simply Click in July Month through sprint but that does not work for me. after that suddenly yesterday 23 august I received a call from sbi card call centre agent that they will apply on my behalf of me after completing the process KYC agent comes and take biometrics and photos after that today I received a message to fill app code or send I go to the website and before filling that app code I downloaded that form where I see dob 09 to 19 and email id xxxxxdxxxx@xxxxx.xxx to xxxxxbxxxx@xxxxx.xxx then I can repeatedly tick mark recertification required but after that agent call me and say this thing later corrected. so, now what can I do?
