ICICI AddOn Card withdrawn ?

I have ICICI Rubyx Credit Card (along w/ Add-on card). Card is expiring this month and got renewed CC pro-actively last month. however, I didn't get the Add On card.
I tried submitting a new request, and it states, Add-On card is not available with this Card Variant.

Anyone has more info on this or any communication from ICICI (may b I missed it earlier)

you can get 3 Add-On cards on your credit card for your blood relatives only.



1.For download supplementary card form, visit icicibank.com then Select form center > Personal banking > credit card > Supplementary card request form

KYC Documents

Photocopy of ID Proof & Address proof of supplementary card applicant

List of Document:

Individuals to whom the consignment may be handed over :

a) Self (consignee)

b) Parents (Mother / Father)

c) Spouse

d) Children above 18 years

e) Grand Parents

f) Parents in Law

g) Siblings (brother / sister) (Above 18 yrs)


2.Original Identification document needs to be checked (at least one document) of the person taking delivery of the card.

a) Ration Card, (Applicable only if it has Photograph of the card receiver)

b) Voters Identification card

c) Driving license

d) Passport

e) Pan card

f) Employers ID card with number when card is delivered at Office address ( Company Name and ID No. should be captured in POD)

g) Pension payment order

h) Freedom fighter pass

I) Valid arms license

I applied Add-On today.
- Called ICICI Customer Care and registered request for Add-On Card. They generated a SR no.
- Went to branch, filled the form, mentioned SR no on top of it; filled the form (need to be signed by primary card holder)
- Took along a passport photo, self attested copy of ID & address for Addon member
- Expected card to be delivered in 7-10 working days
