If I remember it correctly then it is per account. It appears after multiple unsuccessful attempts or fetching without proceeding for payment.

Was there any of your cc bill payment failure in this month? Failed txn also gets counted under 750 CB quota.

No, there was a failure transaction but that was almost 1 month back, and that was credited within 48 hrs. This month there were no failures and on a whole I paid around 20k.
