Got the addon card by May 15. Tried escalating to grievance for change in mobile number without any form as done by @joshuatree.

Got reply by May 22 that physical form is needed for change in mobile number. Replied with scanned copy saying that physical form might have got lost in transit as suggested by @sm9811911

Got reply by May 29 that physical form is compulsary. Then sent physical form through home branch and sent a reply to above email.

After no response, sent new email on June 7. Got an email today that mobile number has been updated.

Able to add in add-on holder's MyCards. Was able to add in add-on holder's UPI as well.
