Are the visits free for the add on card holders?
Yes for Domestic Free for add on and 8 guest access with priority pass.

My primary card for lounge access is Axis Olympus (Citi Prestige) and I have 3 add on cards (2 for my in-laws, 1 for my wife)
In any year, we use about 40 lounge visits between the cards (generally used PP except where they don't accept it)
The actual lounge usage number is much higher but a big chunk of that comes from *G/OWE status.

*G = Star Alliance Gold
OWE = One World Emerald
Like hotel status, Airline status helps with a few benefits on Airlines which are members of the alliance (Air India is a member of star alliance). The key benefits are Checkin at Business class counter, extra baggage allowance, priority boarding and I also get upgraded to business at times.
Airline access either provides better lounges than PP or where they use same lounges, you can avoid the line if Airline provides lounge access (some PP lounges also have reserved area for Airline lounge access pax)

i have used axis magnus card for lounge visit 1+ 2 guests this year and the next day same card used 1+2 guests , i didnt like the food in that lounge and the same day i used burgandy private one card used for 1+2 guests . total 3 +6 guests in this year( 2025)

yes recently i faced the same food is not worthy in some lounges

Most Indian lounges are still better than International lounges, specially ones in Europe!
