Axis Vistara Signature card not being issued

I had 3 Axis cards - FK, Privilege, and Airtel. I closed my Airtel one yesterday to get Vistara Signature. But when I called customer care, they told me that Axis Magnus, select, etc is shown under eligible cards for my account, but not Vistara Signature. Also, when I go and apply for it myself from the Axis website, on entering my customer id and generating OTP, I get an error that there is some issue with OTP generation. What to do? How to get Vistara Signature?
Also, I have SBI Aurum and simplySave. How to upgrade simplySave to Vistara prime?
I am mainly planning to spend 2.5L per year, in rentals

SBI Vistara Prime also needs to be applied separately as a fresh application, it can't be upgraded or flipped from existing CC. You can apply via Sprint or an agent
Axis OTP Generation might be a temporary server problem, else you can apply via agent, preferably through airport kiosks
