How to get a add on card with different number from hdfc Bank

I have recently upgraded at my lifetime Paytm HDFC credit card to 1st year free millennia rupay credit card.

Now I would like to get issued an add on credit card for my brother and would like to registered his personal mobile number to the millennia rupay credit card so he can use it in his upi apps.

He doesn't have any association with HDFC Bank.

Can anyone guide me how to do this?

Thanks for the response. The add-on card request form has mentioned to enter add-on customer id but there is no star for it. Does it means it's not mandatory right?

And can I apply for all 3 add-on cards for my family members in the same form?
