How to make an Add-on card for Axis bank Credit Cards?

I wanted to create an add on card for axis flipkart and myzone
Can I use bogo on movies with MYzone add on card?

I tried to apply an add-on card for my family member after entering his details I got this error "There are multiple customer IDs linked with your details." (As He already has an Axis Bank credit card).

How can you apply add-on for existing credit card user?
