No, add-on cards are free.
₹3000 is for metal card early closure and also for replacement of metal card.
And for plastic card, replacement charge is ₹145.

Try to apply from Browser rather than app. It will mark a reservation.

Yes, for add-on metal card we have to pay 3k, plastic cards are free.

Another con of Federal Onecard is that, they are providing 12% valueback at grocery stores while my father's BOB issued Onecard is having 20% valueback offer. Please correct me if I'm wrong, maybe it's user specific and not related to bank.

Earlier it was possible by enabling mock location. But now the updated app detects it and does not allow to activate the offer.
If you have not updated the app recently, you can still do it.

@Shubham Yadav did the txn at same store for which Offer was activated but he visited different different store same day to use it mutiple times.
