is the add-on member has HDFC account? If so you can do it online from the netbanking by entering the details.

Netbanking-->Cards-->Transact-->Request Add on Credit Card


If non-hdfc, then offline you need to apply, Form --> https://v.hdfcbank.com/content/dam/hdfc-aem-microsites/common-pdfs/pdf/add_on_card_application.pdf

From my Infinia experience, as it is the same Card number in HDFC, it goes to the Primary mobile only(any txn done)

if add-on number is same as primary and linked mobile no is also same, then it seems HDFC add-on card cannot be used online or via UPI and is only useful for POS txn

You surely can't with any of the HDFC Cards.

I am also looking for any Rupay card which can be used with a different mobile number on UPI using the Add-on card.

It is there in the same link you have shared, how to access International Lounge using Rupay cards. I am not sure about add-one ones, as HDFC has the same number on the cards.
