Surname vs Initial

Guys, while booking an award tickt on United Mileage prog, it is asking my 'First Name', 'Last Name' & 'Middle name/Initial' while I only have 'Given Name' & 'Surname' in my passport....so how should i Fill the detials so that my details are captured correctly on the ticket. Plss help. Its urgent.




P.S. I have a 2-worded 'Given name" on my passport and a "Surname" too

Fill accordingly

First Name: Given Name (Enter 2 words in same box)
Last Name: Surname
Middle name/Initial: Keep blank. (If there is * then fill given name 2nd word 1st letter in it, But keep 1st & 2nd word of your given name in first name.)

Another issue similar to above.

I have 2 words under Given Name(Kumar Vishwas) in my passport and Middle Name and Surname fields are empty.

So in United Account and or any other major airline account, Last Name in mandatory.

So what should I enter in that as I think according to rules I should be entering the Given Name in the First Name, so what should be entered in Last Name field so it doesn’t create an issue while booking flights, issuing VISA or during immigration process?

I checked with United customer care and he has added LNU under Last Name field, is that acceptable worldwide looking at my passport where Surname field is empty?
