I really wanted a Reserve for my int'l trip. Despite being a Burgundy Salary holder both my RM and the Axis gentleman here refused to apply saying 4th card will not be approved. I hold Magnus, Select and Atlas. This 3 card rule is very stupid when any which ways they are distributing their overall customer limit across the cards. One can't spend beyond capacity. I am surprised reading this thread on 4th card being processed. Don't know what tier to get into to get a Reserve or a Samsung card if needed without disturbing the other cards.
Let me give you an understanding about the thought process behind this policy.

In Axis there are three reward platform:
1. Edge Reward - Magnus, Reserve, Myzone, Privilege, Select, etc.
2. Edge Miles - Atlas
3. Cashback - Ace, Airtel, Flipkart, Freecharge, etc.
As per them, it's sufficient for every customer to own one card from every reward platform.
Hence they are adamant on providing 3 cards. But for 4th card, some people like
@jack333 got it with luck.
Some other like
@anirban.choudhury got it with approval.
No, one can predict your luck, so better to take approval for avoiding Enquiry.
If your RM/BM is not allowing 4th approval, you either show him some fear of moving your account to other bank/branch, and closing your relations. Negotiations skill cannot be taught straightforwardly.
