Go to branch. Any hdfc branch.
Ask for eAGE form.
Fill your details of the account for which you want card. on the front as well as on back same number.
For the millennium code is VLC50. Its important.
Sign the form and submit.
Mostly they do ekyc but sometimes it doesn't work so they may ask for original AADHAAR PAN. just bring it with yourself they will print it for you.
Ask for eAGE specifically. Most of them don't know it exists.
They don't even know you can take new DC via this form.
All they care about is boring you with insurance fd demat term plan for the time you spend in the branch so BRACE YOURSELF.

If I first upgrade my relationship to classic program and after that i apply for millennia debit card via eAGE form then no charges will be deducted right?
