Was it this form or something else
yes, this form was there

If you're trying to replicate exact scenario of my account ... I'll suggest to just go and give it a try ... if it's possible, then okay, if not, then you'll have to wait and have patience and apply for add-on cards one by one ... I'll be doing the same in case of mother's account but thankfully in my case, it saved 2 trips to the bank

@sahil1033 How to apply for all 3 once? I was told by bank after activating one we can only apply for second and so on...
sheer luck maybe or chances are, I satisfied some internal condition to make this possible ... even I didn't go there to apply add-on cards but out of curiosity asked and was able to apply at once

in my mother's account, it didn't allow to apply for add-on card

so, even I don't have the answer what I did

yes, 3 cards are showing up starting 6524 and ending with different last 4 digits

basically cards have been generated, it's just that I'm yet to receive them :drum:
While applying did u give separate phone numbers for add on or is it not necessary on same number works for 3 different rupay select cards on rupay platform?

at the time of applying for add-on cards, I was asked name, relationship with primary cardholder and DOB ... that's it, no phone number was asked

I went to PNB today ... first of all, I was told that I can't get this debit card as QAB should be above Rs. 50k but I said that code is SAI and it can be done if I've Rs. 1L or above in the account.

So, first, I was told to get KYC done as part of process of getting KYC done at a specific interval time to time. Staff who does KYC wasn't available due to election duty, so, I gathered all the patience in the world and politely tried to get it done from any other staff present. One of the person helped to get my KYC done and after 1.5 - 2 hours, I was able to get my KYC completed and then I filled out the debit card request form.

The lady at the counter mentioned that yes, I can get this debit card and issuance fee is Rs. 708 and I confirmed to go ahead. At that point only I asked to issue add-on debit cards for my family members if it's possible, so, applied for 2 add-on cards as well. Rs. 2,124 has been debited from statement against debit card issuance charges. Now, waiting for all 3 debit cards to arrive. I hope it goes well.
UPDATE (17-Nov-2025) :

Received all 3 DCs (1 primary + 2 add-on) at once today.

Congrats!

Did you get tracking details and link from PNB? Or did it just arrive 17 days later without updates?

Also please confirm: one can register RSDC and all add-ons under one account in Rupay website or is there a limit of cards?
I got tracking number on SMS the day it was booked but in the meantime, I visited branch and had asked for tracking number a week before.

People are saying that one type of card can only be associated to a single RuPay account, so, I'm not even going to try this and waste my efforts. Anyone else can confirm on this? It would be really helpful.
